1. Daily Health Assessment Checklist
 
Please click HERE! You can find a checklist for your daily health assessment and you need to check before you come to church so, you may know whether you are able to come to church, stay home or take a test.

2. COVID Tracking Sheet at all the entrance inside the church building.
    As soon as you get in the church building, please fill in the COVID Tracking Sheet, so we may use it when it’s needed to track some COVID case.
